5. Posology and Administration

The capsules should be swallowed in whole without chewing or breaking. You should not add the drug to your meal or mix it with liquids as it may damage enteric coating.
A recommended starting dose of Duloxetine is 60 mg once a day regardless of meal. In some patients there is a need to increase the dose from 60 mg once a day to the maximum one -  120 mg daily in two intakes, in order to reach a good result. There has been no systematic judgment of the drug administration in a dose more than 120 mg.
In patients suffering from kidney function failure: the starting dose should be 30 mg once a day - in patients with severe failures of kidney functions.
In patients suffering from failure of liver function: the starting dose of the drug should be decreased or the number of intakes in patients with cirrhosis should be cut (to 30 mg once a day).
Age: the drug is recommended for use in patients of ≥ 18 years of age. There is no clinical experience of the use of Duloxetine in patients under 18 years of age.

6. Counterindications

Hypersensitivity, simultaneous administration of the drugs not approved by the attending doctor, closed-angle glaucoma, lactation period, age under18 (there is no experience of the use). It is used with caution in the aggravation of manic condition, epilepsy, mydriasis, intraocular disturbance of pressure, kidney and/or liver failure, in high probability of suicidal attempts and in pregnancy.

7. Should Be Taken with Precautions, if…

In epileptic attacks, hallucinations, disturbances of the function of liver or kidneys, in possibility of suicidal attempts. Care is needed while administration of the drug in patients with manic episodes in their behavior.

8. Interface with Other Medications

If it is necessary to take Duloxetine simultaneously with other medications there is a need of an approval of the attending doctor.

9. Overdosing

There are reported some cases of overdosing in a single-step oral administration of up to 1400 mg of the drug without any fatal consequences. Overdosage may be accompanied with the following symptoms: convulsions, ataxia, vomiting and anorexia.

10. Side Effects

Dizziness, dry mouth, nausea, constipation, sleeping disorders (drowsiness or insomnia) and headache, diarrhea, vomiting, anorexia, weight loss, weakness, blurring of vision. Also low libido in women, retention and disorder of ejaculation, atony of erection in men.
There are reported dizziness, nausea, headache as frequent unfavorable effects in the cancellation of Duloxetine.

11. Pregnancy and Lactation

The use of the drug in pregnancy is possible only in the cases when a supposed favor for the mother exceeds a potential risk for the fetus, as there is an insufficient clinical experience of the use of Duloxetine in pregnancy.
If it is necessary to use the drug in the period of lactation there should be decided an issue of stopping breastfeeding (due to the lack of the experience of use).
Women-patients should be warned that in case of occurrence or planning of pregnancy in the period of use of Duloxetine they must inform their attending doctor.
