3. Indications for Use.

Individual prevention and treatment of all kinds of malaria, extraenteric amebiasis. Chronic and subacute forms of systemic lupus erythematosus, rheumatoid arthritis, dermatoses. Some forms of abnormal heart rhythms.

4. Action

Chingamine (Chloroquine) is fast to provoke the wreck of activators of malaria. It removes rhythm disturbances. The treatment of rheumatoid arthritis results in gradual calming down of pains; stiffness is reduced, and mobility of joints improves. The drug is good and swift to absorb and it slowly evolves out of the body.

5. Posology and Administration

A dose is to be set up individually on the basis of the indication and the patient’s age. It is applied on special schemes.
Orally, for preventive care of malaria -  0.5 g in the first week 2 times, then 1 time always in the same day of week.
In the treatment of malaria Chingamine is to be administered orally (after meal) to adults 2,0-2,5 g for the course of treatment. For the first intake 1 g (4 tablets 0,25 g each), in 6-8 hours - 0,5 g, in the 2-nd and the 3-d days 0,5 g in one intake. Additionally 0,5 g of Chingamine is administered in the 1-st day of treatment, predominantly in tropical malaria. In tropical malaria the treatment may last 5 day; on the 4-th day and on the 5-th one there administered 0,5 g in one intake.
Usually the drug is for oral administration, but in malignant course of malaria the treatment starts from an intramuscular introduction. A single dose for adult is 10 ml of 5% solution; if needed injections are to be repeated with an interval of 6-8 hours, but not more than 3 times (not more than 30 ml).
Intravenous injections are administered only in supersevere cases. 10 ml of 5% solution is dissolved in 10-20 ml of 40% glucose solution of isotonic solution of sodium chloride. The solution is to be introduced to vein slowly.
When the patient’s state is improved injections are to be ceased and the drug is to be taken orally.
In the treatment of rheumatoid arthritis 0,25 g (1 tablet) once a day after dinner 2-3 hours before sleep during 3-6 weeks and in some cases during 3-6 months is to be given.
In subacute course of lupus erythematosus Chingamine is to be administered in the first 10 days  0,25 g 2 times daily (after lunch and dinner), and then 0,25 g once a day (after dinner); in the whole during the course of treatment 70 -100 tablets (17,5-25,0 g) are taken. In acute course of systemic lupus erythematosus Chingamine is to be combined with hormonal drugs: in the first 2-3 days Chingamine is to be taken 0,25 g every night at bedtime, further on (if the tolerability is high) -  0,5 (2 tablets) a day. In the whole for the course of treatment 100-120 tablets are needed (25-30 g). In spring aiming to decrease the events of (hypersensitivity of the body to the sunlight) Chingamine is to be administered preventively: first 1 tablet (0,25 g) once a day, then 2-3 tablets a week.
Sometimes in the quality of anti arrhythmic drug in order to remove failures of heart rate Chingamine is administered orally 0,25 g 2-3 times a day, a dose will be gradually reduced up to 0,25 g once a day. Intravenously the drug is to be introduced in a dose of 0,5 g (with a repeated introduction of 0,25 g) in order to cut short arrhythmia.
The highest doses for adults in oral administration: single-shot - 0,5 g, daily - 1,5 g.

To children on the basis of age the drug is administered in the following doses: the 1-st day to children under one year of age - 0,05 g, from 1-6 years of age - 0,0125 g, from 6-10 years of age - 0,25 g, from 10-15 years of age - 0,5 g; the 2nd and the 3-d day, respectively, 0,025; 0,05; 0,0125; 0,25 g.
For elderly people over 65 years of age the recommendations of Chingamine therapy are approximately the same as for children of early period or these of adolescence.

6. Counterindications

Hypersensitivity, disorders of liver and kidneys functions, frank impairments of cardiac muscle (with rhythm disorders), illnesses of blood and nervous system, illnesses of retina and cornea of an eye, diabetes, chronic alcoholism.

7. Should Be Taken with Precautions, if…

In Chingamine therapy it is necessary to have clinical blood and urine analysis, to control the liver function and at times to have check-ups of eyes.

8. Interface with Other Medications

If you take Chingamine together with other drugs you should take into account the probability of interface of medications. In such cases a doctor’s advice is needed as a first step.

9. Overdosing

Symptoms: headache, impairment of consciousness, vomiting, disturbance of vision, convulsions and coma. Death due to respiratory distress may happen during 2 hours after the intake.

10. Side Effects

Short-term use of Chingamine by mouth in therapeutic doses is usually tolerated without evident side effects. In long-term use it is possible the emergence of dermatitis (skin inflammation). In emergence of dermatitis a dose is to be decreased or the drug should be cancelled. There may emerge dizziness, headache, nausea, sometimes vomiting, noise in the ears, visual distortion. Usually such events disappear by itself. Also there may be observed decrease of appetite, pains in the stomach (due to irritation of mucous coat of stomach); in some patients there is a temporary decrease of the body mass.

11. Pregnancy and Lactation

The use in the period of pregnancy is possible only in case of the drastic need which grade may determine only the doctor. If the use in the period of lactation is needed a question of the cease of breastfeeding should be considered.

12. Driving
