3. Indications for Use.
Anaphylactic shock, hives, hay fever, serum disease, acute vascular purpura (hemorrhagic exudative erythema), hemorrhagic diathesis, vasomotor rhinitis, multiform exudative erythema (acute allergic reaction), Quincke’s disease, contact dermatitis, itching dermatoses, itch, allergic conjunctivitis and other allergic eye diseases, allergic reactions enticed by medications intake. Moreover it is used in X-ray sickness, bronchial asthma (in combination with other medications prescribed by doctor), sometimes in ulcer of stomach and duodenal and hyperacid gastritis, in chorea, in travel sickness, Meniere’s disease (non-inflammatory disease of inner ear), vomiting of pregnant women, after-surgery vomiting, concussion, sunburns and first degree burns, freezing injuries, in cases of allergic reactions on bites of insects, insomnia, neuroses and neurasthenia.
4. Action
The drug is well distributed in the body; it is anti-allergically active, takes local anaesthetic, spasmolythic and moderate ganglioblocking activity (smoothing of painful spasms). Takes sedative and drowsy effect, renders a moderate anti-vomiting action, likewise it has a cholnolythic (in plain - dizzy) activity. The drug starts to act at the end of 1,5 -2 hours after intake, during 24 hours it is completely excretes from the body with urine.
5. Posology and Administration
By mouth, intramuscular, intravenously, per rectum, per skin, intranasal, to conjunctive sack. Orally to adults— 30–50 mg 1–3 times a day, for prevention of motion sickness – 30–60 minutes before travel. In insomnia — 50 mg before going to bed. Maximal single dose is 100 mg, daily dose is 250 мг. An intramuscular dose is 10–50 mg, maximal single dose is 50 mg, and a daily one is 150 мг, intravenous by drop infusion 20–50 mg (in 75–100 ml of isotonic solution of sodium chloride). Maximal dose for adults is 400 mg/daily.
Children before one year of age are administered an oral dose of 2–5 mg, 2–5 years of age— 5–15 mg, 6–12 years — 15–30 mg in one intake. Rectal suppositories are used 1–2 times a day (after depletion).
For children before 3 years of age suppositories, containing 5 mg of diphenhydramine are applied,
For children from 3 to 4 years of age are used 10 mg, from 5 to 7 years of age 15 mg, 8–14 years — 20 mg.; the course of treatment runs 10–15 days. To instill into a conjunctive sack 0,2–0,5% solutions are prepared (better in 2% solution of boric acid) and there are instilled 1–2 drops 2–5 times daily. For local application a thin layer of gel is to be put on afflicted parts of the skin 3–4 times a day, also it is possible to produce and use 3–10% of creams and unctures. In rhinitis – intranasal intake is applied (introduction of liquid directly to nostrils), 50 mg to each nostril.
6. Counterindications
Hypersensitivity, (drug administration during pregnancy is possible only in cases of acute necessity), breastfeeding, childhood (neonatal period and prematurity state)
7. Should Be Taken with Precautions, if…
Angle-closure glaucoma, hypertrophy of prostate gland, stenosing ulcer of stomach and duodenal, pyloroduodenal obstruction (ulcer of neck of urinary bladder), pregnancy. Do not use this drug for the treatment of cold, actually this medication may impair cold or cough by thickening nasal secrecy and drying mucous linings. Care should be taken while using it in patients with recent respiratory diseases in anamnesis (among them is asthma), high intraocular tension, in hyperthyroidism, cardiovascular diseases, hypotension (low level of arterial tension).
Diphenhydramine can provoke negative side effects, especially in people older than 60 years of age. It activates (even in small doses) cortical convulsions on EEG and may provoke an epileptic attack in patients with local brain damage or epilepsy.
8. Interface with Other Medications
Alcoholic drinks and opiates favor enhancement of interface when they are used together with this medication.

9. Overdosing

Distress or excitement of central nervous system in children, mental fog, development of convulsions and lethal outcome.
10. Side Effects
Dry mouth, nausea, vomiting, dysfunction of alimentary system, loss of coordination, tiredness; this medication also provokes drowsiness and dizziness. Side effects will wear off after decrease of a dose or cancellation of Diphenhydramine,
11. Pregnancy and Lactation
The drug should be used with caution and under strict medical control during pregnancy (there are no adequate and strictly controlled studies in pregnant women). Breastfeeding should be ceased during treatment.
