Arterial hypertension, chronic heart failure, condition after myocardial infarction (especially while there are events of heart failure), prevention of unexpected death after myocardial infarction.

4. Action

Ramipril is a drug which reduces arterial tension, narrows down lumens of vessels, and does not substantially change renal blood flow (indeed, in some cases Ramipril enhances it).

5. Posology and Administration

Posology and administration are individual. When there is arterial hypertension the starting doses are 1.25-2.5 mg once daily, supporting ones are 2.5-5 mg once daily, if needed it may be 10 mg, rarer it is 20 mg. In chronic heart failure the starting dose is 1.25 mg once daily, supporting doses are up to 10 mg/daily. For patients who have had myocardial infarction the starting dose is 2.5 mg twice daily, if needed the dose may be further on increased up to 5 mg twice daily, supporting doses are 2.5-5 mg twice daily.

6. Counterindications

Hypersensitivity to the drug, diminution of renal arteries, condition after renal transplantation, diminution of aorta, heart diseases of vague or controversial origin of non-inflammatory nature, the diseases are accompanied with heart increase edemas, liquid accumulation in abdominal cavity, high arterial tension, pregnancy, breast-feeding period, severe compromised renal function, liver failure. Ramipril should not be administered to children of less than 14 years of age.

7. Should Be Taken with Precautions, if…

The treatment is carried out under a regular medical control. For patients with concurrent offences of renal functions the doses should be chosen individually in compliance with CK indications. Before the start of the treatment all the patients should have had function study of kidneys.

9. Overdosing

Symptoms: acute arterial hypotension, stroke, angioneurotic shock, myocardial infarction, thromboembolic disorders.

10. Side Effects

Hypotension (drop of arterial tension), collapse, (acute drop of arterial tension), disorders of heart rhythm, aggravation of ischemic disease of the heart and the brain, compromised kidney function to the extend of progression of acute renal failure (especially if diuretic agents are taken simultaneously), proteinuria (albuminuria), nausea, diarrhea or constipation, vomiting; neurologic disorders and others.

11. Pregnancy and Lactation

Counterindicated in pregnancy. During treatment breast-feeding should be stopped.

12. Driving

Patients whose activity requires quick attention concentration and prompt response (vehicle drivers, machinery operators etc.), should take caution while Ramipril therapy.
